Our care managers are all about taking care of you—with knowledge, experience, understanding, and sensitivity. Focusing on your needs, they help you assess, plan, coordinate, and manage all aspects of your care. They provide access to reliable in-home care, accessible home modifications, and specialized equipment designed to make your life easier. They work with you to create an independent living plan, assist in its implementation, help determine if financial assistance is available, and guide your progress for as long as you desire. Your care manager will be at your side on the road to more independent living.

We have over 40 years of experience in managing care plans and coordinating services to help individuals with physical and age-related disabilities live independently in the community. Let us help you live safely and independently at home.

 

Supports Coordination

Our customers are more than just a name. We value building true relationships. You will be assigned a dedicated Supports Coordinator who will help you achieve your goals. Your personal Supports Coordinator can help you with:

  • Developing your service plan.
  • Finding personal care attendants and respite services.
  • Collaborating with financial management services for the consumer-employer.
  • Coordinating home/environmental modifications, personal emergency response systems and other medical equipment.
  • Monitoring your plan to maximize services.
  • Connecting you to community resources.
