Nursing Home Transition Program: for nursing home residents returning to their homes, up to $4,000

PDA Waiver: Medicaid eligible and nursing facility clinically eligible, funding limits made on a
case-by-case basis

PHFA Loan Program
Up to $10,000 for home modifications and up to $2,000 for closing cost and down payment assistance for low to moderate income people with permanent disabilities

USDA Development Section 504 Loans and Grants
Covers most of Lancaster County outside Lancaster and Columbia cities, for median income, or less, property owners.  Maximum loan: $20,000; Maximum grant: $7,500.

Provides financial assistance and loans of actual equipment for people with disabilities needing assistive technology or home modifications
