The word CHOICES is an acronym for Creative Holistic Opportunities Individualizing Community Educational Services.

CHOICES is a life skills day program for mentally alert adults with physical and other disabilities.  The program's goal is to touch individual lives in a way that enhances self-esteem, confidence, independence, decision-making skills and encourages personal growth.

In order to attend CHOICES a person must be age 18 or older, have a developmental disability, be cognitively able to indicate choices and participate in daily programming, be able to make personal care needs know and be able to self-medicate.

CHOICES is supported through community donations, the fund raising efforts of United Disabilities Services, and monthly participant dues. Program participants are charged monthly club dues based upon the frequency each individual attends CHOICES. Individuals who are financially unable to pay the established dues may apply for a reduced fee. All program participants are required to pay something toward the cost of the program.
