ACCESS Home Modifications values the partnerships we have created with several
local organizations that have goals and missions that complement our own. UDS has assembled a county-wide collaboration of partners, many of which had no history of working together, to create innovative accessible housing solutions for Lancaster
County that none of the partnering agencies could have achieved while working alone.
PA Medicare Waiver Programs
Roles: Waiver Funding and Referral
Mission: waivers help people avoid institutionalization and remain in their own homes
UDS manages the PA Medicare Waivers for Lancaster and Lebanon Counties. The Waiver Specialists and the ACCESS manager meet on a regular basis to share resources. There is a mutual referral process, and we regularly cost share projects to spread home modification costs across all appropriate funding streams.
Lancaster County Office of the Aging
Roles: Waiver Funding and Referral
Mission: to enhance the quality of Life of Older Lancaster County Residents by empowering diverse communities, the family and the individual.
UDS receives regular referrals from the Office of Aging (OoA) and works with OoA on the Nursing Home Transition Initiatives. Currently, UDS is the subcontracted provider of Home Modification services for the Office of Aging in Lancaster County.
Lancaster County Housing and Redevelopment Authority
Roles: Funding (HOME, CDBG, PHFA) and Referral
Mission: To administer programs to carry out housing and homeless assistance activities and community planning and economic development activities.
The Lancaster County Redevelopment Authority is currently the applicant of record for our ACCESS Home Modification Program in Lancaster County. As part of this arrangement, we have a mutual referral process to assure appropriate use of funds for both agencies. Once our program is transitioned to PAHP funding, this cooperative approach will be continued, using the same personnel liaisons already established.
City of Lancaster
Roles: Funding (HOME, CDBG, PHFA) and Referral
Mission: to promote reinvestment in the existing housing stock in Lancaster City, preserve existing housing through maintenance and rehabilitation and increase homeownership
The City of Lancaster is currently the applicant of record for our ACCESS Home Modification Program in the City of Lancaster. As part of this arrangement, we have a mutual referral process to assure appropriate use of funds for both agencies. Once our program is transitioned to PAHP funding, this cooperative approach will be continued, using the same personnel liaisons already established.
Lancaster disAbled for Change and Justice (LdACJ)
Roles: Consumer Direction and Referral
Mission: to promote positive changes in our community to improve the quality of life for persons with disabilities.
LdACJ will provide in-depth knowledge regarding the identification of special issues and needs of people with disabilities. Besides referrals, their members will assist the program administrator with the development of the project work specification sheet.
LdACJ is an advocacy group organized by persons with disabilities.
Disability Empowerment Center
Roles: Consumer Direction and Referral
Mission: to advance the rights of persons with disabilities through the elimination of barriers that people with disabilities experience.
The Disability Empowerment Center (part of the state-wide system of Centers for Independent Living) empowers people with disabilities to fully participate in all aspects of society. The Disability Empowerment Center offers services to people with disabilities as well as families of individuals with disabilities, community service providers, government agencies, private businesses and the community at large.
