This state funded program offers personal care attendants, payroll for your attendant and supports coordination. Similar to the Attendant Care Waiver program, you can access services if you are an adult with a physical disability. This program differs from the Attendant Care Waiver program in the following ways 1) you do not have to meet the financial eligibility requirement for Medical Assistance and 2) you do not need to meet the Nursing Facility Level of Care that is required for the Attendant Care Waiver program.

In the Act 150 program, applicants whose family monthly gross income exceeds 300 percent of the Federal Benefit Rate, (as published annually in the Federal Register) will pay a fee, based on a sliding fee scale set by the state. Within this fee calculation, family expenses related to medical care or disability related care is adjusted in determining an applicant’s fee. Your UDS Supports Coordinator can help you determine the fee for services.
