Community of Choice Housing Grant

 

This is a service made possible by a grant from the Pennsylvania Developmental Disabilities Council. The goal of the Community of Choice Housing Grant is to provide young people assistance in developing a support network to help them examine where they would like to live. The network, which is called "Circles of Support" creates goal-oriented and person-centered plans.

The focus is to help each individual gather the skills and resources necessary to make a successful transition to the home of their choice and become an active member of their community.

Counties served
Berks, Lancaster, Lebanon, York

Eligibility
Any student 16 - 21 with a disability is eligible. Students have an Individualized Education Plan or receive services through the Lancaster/Lebanon IU 13. The program can help students plan where they want to live in the future.
