Our care managers are all about taking care of you – with knowledge, experience, understanding, and sensitivity. Focusing on your specific needs, they help you assess, plan, coordinate, and manage all aspects of your care. They provide access to reliable in-home care, accessible home modifications, and specialized equipment designed to make your life easier. They work with you to create an independent living plan, assist in its implementation, help determine if financial assistance is available, and guide your progress for as long as you desire. Your care manager will be at your side on your road to more independent living.
