Pennsylvania State Waivers & Financial Assistance Program

Below is a quick reference of some of the Pennsylvania State Waivers and financial assistance program. In addition to the qualifications below, participants must be a U.S. Citizen or qualified non-citizen and a resident of Pennsylvania.

Attendant Care Waiver Between 18 – 59 Needs basic personal attendant services Nursing facility clinically eligible (assessment required) Long term Physical Impairment (12 months or more including terminal illness) Mentally alert enough to direct services (selecting, supervising attendants & managing their own financial and legal affairs); cannot  be ventilator dependent at time of enrollment Meet requirements as determined by County Assistance Office
Act 150 Program(Provides same services as Attendant Care but not funded through Medical Assistance) Between 18 – 59 Needs basic personal attendant services Nursing facility clinically eligible (assessment required) Physical Impairment  (expected to last for a continuous period of not less than 12 months) Mentally alert enough to direct services (selecting, supervising attendants & managing their own financial and legal affairs); cannot be ventilator dependent at time of enrollment May be assessed a minimal co-payment. This co-payment is based on participant’s income and will not be more than the total costs of services
Adult Autism Waiver Age 21 or older N/A Eligible for Intermediate Care Facility (ICF) or Other Related Conditions (physical, sensory or neurological disabilities which manifested before age 22 and are likely to continue indefinitely) N/A Diagnosis of an Autism Spectrum Disorder which includes: Autistic Disorder; Pervasive Development Disorder, Not Otherwise Specified; Asperger Syndrome; Childhood Disintegrative Disorder; Retts Disorder Meet requirements as determined by County Assistance Office
OBRA Waiver Disability occurred before the age of 22 – still under age 50 at time of enrollment Must have at least 3 of 6 *functional limitations Eligible for Intermediate Care Facility (ICF) or Other Related Conditions (physical, sensory or neurological disabilities which manifested before age 22 and are likely to continue indefinitely) Likely to continue indefinitely Must have severe developmental physical disability; need for high level of habilitation services Meet requirements as determined by County Assistance Office
COMMCARE Waiver Between 21 – 59 at time of enrollment Must have at least 3 of 6 *functional limitations Nursing Facility clinically eligible (assessment required) Likely to continue indefinitely Must have traumatic brain injury diagnosis and need for high level of habilitation services; cannot be ventilator dependent at time of enrollment Meet requirements as determined by County Assistance Office
Independence Waiver Be 18 – 59 at time of enrollment Must have at least 3 of 6 *functional limitations Nursing Facility clinically eligible (assessment required) Likely to continue indefinitely Physically disabled; may not have mental retardation or have major mental disorder as primary diagnosis Meet requirements as determined by County Assistance Office
PDA/Aging Waiver Age 60 or older at time of enrollment N/A Nursing Facility clinically eligible (assessment required) Likely to continue indefinitely Must have a physical disability Meet requirements as determined by County Assistance Office

* The 6 Functional Limitations Are: Self-Care, Communication, Learning, Mobility, Self-direction and Independent Living
