To Submit A Job Application

You are about to begin the process to submit an online employment application to UDS Foundation.

This includes positions for any of the following companies:  Accessing Independence, Independent Living Services, Adult Enrichment and United Disabilities Services.

To get started, we will be collecting some basic information about you as it relates to the position you are applying for as well as completing three forms that we need to obtain from all applicants to comply with State and Federal regulations.

Once you have completed this short application, you will have the opportunity to tell us more about yourself and your qualifications by emailing us your resume.

A few things to know before you begin:

  • There are 6 pages to this application… but don’t worry, it shouldn’t take long for you to complete. Some of the pages only have one question on them.
  • Click on the “Next Page” button at the bottom of each page to continue through the application.
  • The areas where you can type are highlighted in gray.  Click on the gray boxes and enter your answers. Make sure you have addressed all of the gray shaded boxes before moving to the next page.
  • Once you have landed on the 5th page, read the information at the top and click the submit button to send us your form.
