Dear UDS Staff:

UDS cares about its employees and we're continually making improvements to make UDS a great place to work. This year, with the approval and support of the UDS Senior Management Team, a Wellness Committee has been established to develop a workplace Wellness Program. The idea is to work in a setting that will support the values of choosing a healthy lifestyle.

We need your help and ask that you complete the employee survey listed below. The success of the program depends a great deal on how well it meets your needs. If you completed a survey at our Wellness Day on September 20th, there is no need to complete another form. Thank you.

Although a healthy lifestyle is a personal choice, this program will provide you with informaiton that can make it easier for you to make healthy choices. We encourage you to take advantage of this opportunity to its fullest.

Thank you very much in advance for helping us launch our workplace wellness program. This is a great opportunity for all of us to come together.

The Wellness Committee

Please answer the following questions as honestly as you can. You do not have to share your name, all answers will be compiled as a group.

1. Would you buy healthy snacks if available? Examples: pretzels, cereals, yogurt, 1% or skim milk, fresh fruits, 100% juice, dried fruits.

2. Would you use your break for a personal activity? example: stretching, yoga or a walk, if there were a place to do it?

3. Would you participate in group activities encouraging healthy eating or physical activity if there were offered to staff?

4. Are you satisfied with your current state of health?

5. Do you make time for at least 20 minutes of physical activity 3 times a week?

6. Do you think about your health when deciding what to eat?

7. Do you smoke cigarettes?.
Are you interested in trying to quit?

8. Are you more than 10 pounds over your ideal weight?

9. Do you know what your blood pressure is or what it should be?

10. Are you concerned about reducing your level of mental stress?

11. Do you know what your cholesterol level is?

12. Do you have periodic bouts of low back pain?
